Most applications come from a parent or guardian filling out on behalf of the traveler. To avoid any confusion, we ask for both.

Which trips interest you?

Pick one or more — applying for multiple trips is encouraged. Or check “Open to suggestions” if you’d rather Scott recommend a fit.

If accepted for more than one trip you’ll be invited to confirm any combination — applying broadly doesn’t commit you to anything.

Medical basics.

High-level only. Detailed medical paperwork happens after acceptance — and it’s the same form your medical team already uses, mirrored from the Frontier app.

All medical fields encrypted at rest. HIPAA-eligible storage. Only Scott + the trip’s medical lead see this.

Daily life & history.

A clearer picture of how the traveler navigates the world day to day. Helps Scott match them to the right trip + the right roommate pairing.

Same HIPAA-eligible storage as the medical fields. Honest answers help us prepare; nothing here disqualifies anyone.

Behavioral & social profile.

These questions help us pair travelers safely with roommates and give staff the context they need to support the traveler well. We don’t share any of it with other travelers.

The more honest, the better the trip. Frontier’s job is to make travel work — not to judge.

Family & guardian.

Required for emergency contact. If a parent or guardian helps with medical paperwork or payment, they get app access too.

Two people who know you well.

Family friends, current/former teachers, doctors, employers, day-program staff, or other adults who can speak to your daily life. Optional — Scott will also follow up with you directly during the review, so feel free to skip these and share them later by phone.
